Office hours are from 8:00 am to 5 pm. If you are experiencing an emergency or need immediate attention, please call 911 or go to your nearest hospital emergency department.

Please notify our office at least 24 hours in advance if you are unable to make your office appointment. Canceling an office appointment less than 24 hours before the appointment will result in a $25 fee. Endoscopic procedures cancellations require at least three business days before the procedure. A cancellation with less than three business days notice results in a $100 fee.
An office visit is a face-to-face consultation with one of our providers. There will be no procedure performed at the time of a regularly scheduled office visit.
For office visits, please bring the following:
- All your medications including over-the-counter medications
- Driver’s license
- Insurance card(s)
- Any recent lab work reports or x-rays.
- If required by your insurance, you may need to bring a referral from your primary care physician.
Please bring your co-pay. We accept most credit cards.
We accept Medicare, most HMO’s, most PPO’s and other insurance carriers. Please check your insurance company’s provider directory to see if your physician is listed.
Prescription refill requests are taken Monday through Thursday, from 9 am to 4 pm. Requests should be made at least one week before your supply is gone. Prescription refill requests can also be received by visiting our Patient Portal here.
Please provide the pharmacy name, phone number, prescription number, drug name and dosage and how often you take the medication. In the near future we will be rolling out our e-prescription (Electronic Prescribing Program). This will allow us to get your prescriptions to the pharmacists electronically, safely, instantly but will require you to call the pharmacist directly. The pharmacy will then contact us electronically. When we start this, all refills requests will need to be called to your pharmacy.
Please note that we do not refill prescriptions in the evening or on weekends. We also cannot refill prescriptions that were originally prescribed by other physicians.
